About Five Points Healthcare

Five Points Healthcare is now part of the Aveanna Healthcare family of companies, one of the largest providers of homecare in the nation and a leading provider of home healthcare and hospice services. Based in Atlanta, Georgia the Company was founded in 2011 by a team of seasoned home health professionals who had a vision to build a new healthcare company that would successfully blend their diverse experiences and create a premier home health and hospice organization.
