In addition to the core Medicare-certified services, Five Points offers the following clinical specialty programs:
Care Transitions – designed to improve the transition process as a patient moves from an inpatient setting, such as a hospital or rehabilitation facility, back home. Through our Care Transitions we work closely with patients to assist in managing their disease process or rehabilitation with a goal of reducing the risk of the patient being re-admitted back to an inpatient facility.
