Five Points Healthcare is a leading provider of home healthcare and hospice services. Based in Atlanta, Georgia the Company was founded in 2011 by a team of seasoned home health professionals who had a vision to build a new healthcare company that would successfully blend their diverse experiences and create a premier home health and hospice organization. The goal of the founding members was to combine the knowledge and expertise they gained working with large, national home health and hospice companies with the personal touch, focus on patient care and commitment to employees they experienced working with smaller, locally owned companies. The result was the creation of Five Points Healthcare; a company that possesses industry leading experience yet maintains a small-company feel that is committed to its five key stakeholders – PatientsEmployeesPhysiciansProvidersCommunities.

Five Points Healthcare believes that most people prefer to receive care at home whenever possible and patients deserve high-quality options which will allow them to remain independent in the comfort of their own homes. The Five Points senior leadership team has spent their entire careers in the home health setting and is committed to providing the best solution to ensure patients receive the quality care they deserve.

Leadership

Rob Radics, CEO

Mr. Radics founded Five Points Healthcare in 2011 and serves as its President and Chief Executive Officer. Mr. Radics began his career in the home health industry in 1996 with a locally owned agency in Georgetown, SC. He later went on to serve as Executive Director of a multi-location home health company in Arizona and California before relocating to his hometown of Atlanta where he spent nine years with Healthfield, Inc. which was acquired by Gentiva Health Services during his tenure. While with Gentiva Mr. Radics held responsibility for their Georgia operations along with a portion of their Alabama operations. He was also responsible for the integration of multiple acquisitions and the establishment of new start-ups. Mr. Radics obtained a Bachelor of Science in Business Administration while majoring in Health Care Management at Appalachian State University and holds a Masters in Health Services Administration from the W.P. Carey School of Business at Arizona State University.

Anthony Angelo, President – Millenium

Anthony P. Angelo is an executive leader with more than 20 years of experience impacting the reputations, performance and growth of both national and regional home health care companies. He has been with the Millenium Family of Companies for the past 8 years prior to this; he served as Corporate Vice President of Home Health Operations for AseraCare Home Health, a division of Golden Living Enterprises where he oversaw 17 individual Medicare home health agencies in six states. Prior to AseraCare, Mr. Angelo was Chief Operating Officer/Senior Vice President of Clinical Operations at Home Health Corporation of America which comprised 19 Medicare-certified home health agencies in Pennsylvania, Delaware, Maryland and throughout Florida. There he had previously held the positions of Senior Vice President of Compliance and HIPAA Privacy, and Security Officer. Anthony Angelo holds a Bachelor of Science degree in business management from LaSalle University, and is certified in Healthcare Compliance from the Healthcare Compliance Association.

Bob Garvin, Executive Vice President Business Development

Mr. Garvin joined Five Points Healthcare in 2014 and serves as the Vice President of Business Development. Mr. Garvin has over thirty years of experience in sales, marketing, contracting, sales leadership and business development. He began his business career with Dow Chemical in 1984 and has since then served at executive levels with Sage Products, Inc., VinCom, Gentiva, Covidien and Georgia Pacific. During his career he was credited with success in producing unparalleled growth in revenue and profitability through the development of high performance sales organizations. His responsibilities also included successfully launching new products, integrating several mergers and acquisitions and launching several start-up ventures. He is a graduate of Furman University where he received a degree in Business Management.

Deb Erickson-Louchart, RN, Vice President Clinical Services

Deb joined the Five Points Healthcare team in 2017 to oversee clinical care and quality initiatives. She began her nursing career in the public health arena in 1982 and moved into the home health in 1986 when she was appointed to the position of Director of Community Health Nursing and Home Health Agency for the South Dakota State Department of Health. She continued growing in her home health profession upon moving to Georgia in 1990 working as a Patient Care Coordinator and Administrator in what began as a small startup Agency and grew 650% over six years. She was promoted to Senior Vice President of Operations. The Company was acquired by Amedisys and she remained there until 2003. Deb advanced in the Home Health Industry leading a few additional companies and expanded her experience to include Hospice in 2014. From small local companies to national corporations with responsibility for multi-state territories, she has always worked to improve existing operations by promoting patient care excellence, expand geographically through acquisition and transition of additional agencies, and providing consulting services while maintaining focus on the patient. Deb is a graduate of South Dakota State University with a Bachelor of Science degree in Nursing.

Mary Wollstein, Senior Vice President Strategic Initiatives

Ms. Wollstein was instrumental in helping to launch Five Points Healthcare and complete its first acquisition. She joined the Company as its Vice President of Financial Operations in February 2012. Ms. Wollstein has over 27 years of experience in the home health industry serving in a variety of roles. She began her career with Coosa Valley Home Health in 1985 working in the billing department and through multiple promotions ultimately became the Company’s Chief Financial Officer. In 1999 Ms. Wollstein joined Healthfield, Inc. where she served in a variety of roles including Vice President of Mergers and Acquisitions. During her tenure at Healthfield as VP of Mergers and Acquisitions the Company completed multiple acquisitions from 2003 to 2006 at which time it was acquired by Gentiva Health Services. She continued her career with Gentiva as Vice President of Business Initiatives where she led a variety of strategic initiatives including information systems, reimbursement, and clinical operations. Ms. Wollstein holds a Bachelor of Science in Management from Berry College in Rome, Georgia.

Perri Southerland, Senior Vice President Financial Operations

Ms. Southerland has over 30 years of home care industry experience. She began her career working for a Medicare Fiscal Intermediary responsible for auditing cost reports. Using this experience and sound understanding of home health reimbursement and accounting practices she has worked in a variety of roles for home health companies specializing in cost reports, financial oversight, budgeting and CMS fiscal policy. Ms. Southerland holds a Bachelor of Science in Accounting from Auburn University.

Shane Brinkerhoff, President – Five Points

Mr. Brinkerhoff began his career in 1992 in respiratory therapy and in 1996 began working in the homecare industry. He has held several leadership positions in his career among a diverse mix of post-acute providers including respiratory, HME, infusion pharmacy and home health. Prior to joining Five Points Healthcare, he served as Vice President of Multi Area Operations at Gentiva Health Services. A native of southern California, he earned his degree in respiratory care in 1996 at California College for Health Sciences and holds both CRT and RRT credentials by the National Board for Respiratory Care.
